Minneapolis Kids provides year-round, fee-based school-age childcare for families with youth enrolled in High Five/Minneapolis Kids Jr through 5th grade during the school year (entering grades K-7 during the summer).

This presentation will discuss what advance care planning is and what health care directives can do. We will explain who can be a health care agent and the responsibilities of that role. Examples of health care directives and important considerations will be shared. We will not complete health care directives as part of this presentation, but you will be equipped with the knowledge, resources, and where to go for help to complete your directive.
We serve older adults, persons with disabilities and all those who care for them through an integrated system of information, assistance, education, services and facility-to-home support.
